We require the patient’s contact details, the clinical question, medical history and medication details, as well as your contact information. You can upload supplementary findings and documents using our form.
Are short-notice appointments available for acute symptoms?
For acute symptoms, please contact us briefly by phone. Our team will look for a prompt solution and check the available capacity.
Will I receive a medical report after the examination?
After the assessment or treatment, we will provide a prompt update with the key findings, our assessment and further recommendations.
